Differential Diagnosis

History: A middle-aged lady presented with a melanonychia striata (longitudinal melanonychia) involved the peripheral zone of the ulnar side of the index finger with an irregular enlarging brownish macular hyperpigmentation of the proximal nail fold (Hutchinson’s sign) of many months duration. Punch biopsy of the nail matrix at the involved area revealed acral lentiginous melanoma.
