Differential Diagnosis

History: 38 year-old gay male presented with cough and skin lesions on face. Small flesh-colored, umbilicated papules resemble molluscum, but biopsy showed gelatinous type of cryptococcosis with innumerable organisms. Also: chest x-ray positive for nodular infiltrates, consistent with fungal infection. Patient refused bronchoscopy, but sputum subsequently grew C. neoformans. Patient refused therapy and left hospital against medical advice; lost to follow-up. Hospital screening admission labs +HIV.
