Two asymptomatic yellowish cysts in the scrotal wall of many months duration. Three differential diagnoses were put in mind: epidermoid cyst, pilar cyst and steatocystoma. Although, therapeutically whatever the diagnosis all share same therapeutic option (surgical removal) however academically skin biopsy may put the final diagnosis and it was epidermoid cyst in the presented case.
