Differential Diagnosis

History:

66 year-old, febrile and minimally mobile after cervical spinal fusion surgery, was sent to dermatology because intense pruritus on the back. Examination reveals widespread erythema with a few pustules. KOH prep is positive for candida (pseudohyphae and budding yeasts). Treatment with fluconazole (oral) and ciclopirox olamine lotion (topical) was successful. There was not recurrence as patient became afebrile, felt better and became more mobile.
