History:

 

Patient of Dr S Malik

45 yr M , presented with 2 month h/o a painless ulcer on right lateral aspect of knee. Farther questioning reversed 4 yr h/o feeling of numbness in hands/feet and recurrent ulceration due to trauma.
O/E - non tender ulcer on right lateral aspect of knee and a small one on right heel . Loss of sensation -touch and pain tested - on both Feet up till level of thigh and hands up till upper arm- Gross enlargement of Greater auriicular b/l ( seem in photograph ) ulnar , radial , median , perineal and post tibial - Shins were icthyotic , shiny with loss of hair - no patches , plaques
A clinical dx of Leprosy was made - we all were v excited as the patient had been misdiagnosed since last 4 yrs
Slit skin smear stained with Fite stain showed multiple bacilli - biopsy awaited
