History:

Patient of Dr J Lim

70 yr old man, having red non scaly circular rash over both lower limbs then spread to trunk, forearms, and face.

having reduce sensation over the lesions and distal hand and foot numbness.

did slit skin smear and ZN stain, the last photo is thru microscopy from the SSS done over his chest lesions.

 

Multibacillary leprosy

bacteriological index: 3.0
morphological index: 2.4

started on MDT: rifampicin 600mg stat and monthly
,clofazimine 300mg stat and monthly, clofazimine 50mg daily and dapsone 100mg daily for 1 yr.
also given prednisolone 20mg daily for 1 week then tapering weekly to reduce risk of ENL.
other lx: FBC/RFT/LFT/RBS/G6PD/UFEME/ skin biopsy for HPE and mouse-foot-pad for antibiotic sensitivity.
